Kuesioner ini dalam dua bahasa, yaitu bahasa Indonesia dan bahasa Inggris, karena laporannya harus ditulis dalam bahasa Inggris, tetapi disebarkan kepada karyawan yang berbahasa Indonesia. Kuesioner ini digunakan sebagai instrumen untuk mengukur kepuasan kerja (job satisfaction), kompensasi (compensation), dan produktivitas (employee productivity)

Operasional variabelnya sebagai berikut:

Job satisfaction is how people feel about their jobs and different aspects of their jobs. It is the extent to which people like (satisfaction) or dislike (dissatisfaction) their jobs. Job satisfaction will be measured by using the factors influenced it, they are:
  1. Frustration and alienation
  2. The nature of technology
  3. The nature of supervision
  4. Work and psychological well-being
  5. Role incongruence and role conflict
    Compensation program is all forms of pay or rewards going to employees and arising from their employment, in which the major component of it are direct financial payments (pay in the form of wages, salaries, incentives, commissions, and bonuses) dan indirect payment (pay in the form of financial benefits such as insurance). Compensation will be measured by using the components of it, they are:
    1. Base salary
    2. Indirect monetary compensation (benefits)
    3. Equalization benefits
    4. Incentives
      Employee productivity would be measured by outcomes that can be achieved by employee in which qualitative or quantitative work.
